Our home health program is designed
for patients with doctor's order to receive care in their
homes, and for seniors.
Working with your physician, a
personal plan is designed to meet the needs of the client. Such
as, wound care after major surgery, skilled nursing care when
facing long term sickness, and more.
Medical Care Management
Our care team provides direct client
help and support. Such as: ongoing health and wellness review;
Nutrition counseling; Wellness checks; physician and family
communication.
Therapy
Should
clients require physical, occupational or speech therapy, First

Personal Care Assistance
(sometimes called "community care") This program supports
clients who have chronic or long-term disabilities who need help
with routine personal care. These services include help with
personal needs, such as bathing, general hygiene dressing,
grooming. Others include shopping, light housekeeping
activities, medication supervision, all of which help enable the
client remain at home, regain and/or maintain their
independence.
