Who may benefit from home care?
People of any age with medically
necessary home health care needs may benefit from home
health care. This might include:

people recently discharged
from the hospital or nursing home in need of continued
care,

people receiving treatment
for cancer and other chronic illnesses,

people in need of
physical, occupational or speech therapy,
-
Individuals who need
assistance performing activities of daily living, a
result of advanced age, infirmity, or physical or mental
limitations.

How do I
begin receiving care?
Patients can be referred from a physician, a hospital,
nursing home, family, friends or a self-referral.
Please call
our main line (832) 595-0336 and
ask to speak with our intake coordinator. Our intake
coordinator will be happy to assist you in arranging for
home care services. We can also make a call to your
physician to help assess your home healthcare needs

How do patients pay for
home care services?
Payment for home care comes from many sources including
Medicare, Medicaid and private insurance. We provide care
to anyone in need of medically necessary home care,
regardless of their ability to pay. This is made possible
by the generosity of the towns we serve, businesses and
individual donors.
